Pain in the penis during erection, intercourse or at rest is common and almost always has a findable cause. It is worth examining rather than waiting out.
- Peyronie’s disease — pain with erections in the active phase, often with a new bend (more on Peyronie’s)
- Foreskin problems — tightness, splitting or recurrent inflammation

Assessment is a proper examination, urine and swab testing where indicated, and penile Doppler ultrasound if curvature or vascular disease is suspected. Sudden severe pain, or an erection lasting more than four hours, needs urgent attention rather than an appointment.
